In fact, I had a pretty good feeling that's how it would end. About halfway through the book I thought, "Anna's going to die!" Then I thought, "No, that would make too much sense, and everyone said the ending was really surprising." So I kept thinking of other batshit ways that it could end:
-Campbell's service dog is actually to keep him from MURDERING people, so he MURDERS Anna so that she can give Kate the kidney, and Kate ends up dying anyway. But Julia saves the day because she is actually a fairy princess.
-All that talk about stars and astronomy? At the end, Brian reveals that his long shifts at the station have actually been spent crafting a rocket to outerspace, and with Jesse's help (rocket fuel) he jettisons his family into space so that they can all be together looking at the stars when Kate dies.
-They're all actually aliens.
But I kept coming back to, "No, Anna HAS to die, it's the only way it makes sense, it's the only way for the ending to be anything but anticlimactic. Because the whole book has been 'Kate, Kate, Kate' and you notice that Kate never has a segment from her point of view? That's because she's going to have one at the END after Anna dies!"
And I kept saying, "NO, NO, THAT CAN'T BE IT, IT'S WAY TO EASY TO GUESS."
I guess what I'm saying here is I'm a morbid fuck?
